Grace Li, MHA
Chief Executive Officer
Grace Li joined On Lok in 2001 with expertise in orchestrating new site openings, managing implementation of software applications, and overseeing multi-site complex systems and health care programs. In 2016, Grace went from being Chief Operating Officer to being named Chief Executive Officer.
Grace is currently working to build a new senior leadership team, helping to create new opportunities to partner with philanthropic communities, and investing in On Lok’s workforce so it upholds its reputation as a leading employer in senior care. Grace’s vision will build on the legacy of On Lok’s success by expanding on existing core programs in order to continue supporting seniors while keeping up with new needs that arise in an ever-changing world.
Building on more than 25 years of healthcare experience and 15 years at On Lok, her experience spans a wide range of healthcare settings. Grace has worked in hospitals and managed physician practices, assisted living facilities and health care consulting.
Grace serves locally on the Eden Housing, Inc. Board of Directors, NICOS Board of Directors and nationally on the National PACE Association (NPA) Board. She also serves locally on a number of advisory committees that help shape the services and care in our communities (i.e., SF Palliative Care, SF Post Acute Care Advisory committee) Grace received her Master of Health Administration from the University of North Carolina and Bachelor of Arts in Economics from the University of Florida.
Eileen Kunz, MPH
Chief of Government Affairs and Compliance
Eileen Kunz is the Chief of Government Affairs and Compliance for On Lok. In this role, she is responsible for government and community affairs, regulatory affairs and compliance and health plan services departments for On Lok Senior Health Services. Eileen Kunz joined On Lok in 1986 as On Lok Replication Project Coordinator and was involved with the direction of On Lok’s national replication of the Program of All-inclusive Care for the Elderly (PACE) model since its inception. She was instrumental in the formation of both the National PACE Association.
Prior to joining On Lok, Eileen worked for Mills-Pennisula Senior Focus and USDA, Food and Nutrition Service in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC).
Eileen is a member of the National PACE Association Public Policy Committee. She also serves on a number of state and local advisory committees related to improving services for seniors and persons with disabilities including the California Olmstead Advisory Committee, the San Francisco Long-Term Care Coordinating Council and the Santa Clara County Long-Term Services and Supports Integration Campaign.
Eileen attended the University of California at Berkeley where she received an undergraduate degree in Social Welfare and went on to receive her master’s degree from the UC Berkeley School of Public Health. In 2009, Eileen received the Judy Baskins Volunteer Leadership award for her service to the National PACE Association in advancing the PACE model of care.
Jay Luxenberg, MD
Chief Medical Officer
As the Chief Medical Officer, Jay Luxenberg, M.D., provides medical leadership for the five On Lok corporations both internally and externally. He is responsible for medical policy and oversees all medical affairs and clinical functions for both employed and contracted medical providers. Dr. Luxenberg’s post-graduate training in geriatrics was through a fellowship at Mount Zion Medical Center in San Francisco and a Medical Staff Fellowship at the Section on Brain Aging and Dementia at the Laboratory of Neurosciences of the National Institute on Aging, National Institutes of Health. Dr. Luxenberg completed training in Internal Medicine at Mount Zion Medical Center in San Francisco and he graduated medical school at age 22 from the 6-year combined biomedical program of Albany Medical College and Rensselaer Polytechnic Institute. Dr. Luxenberg’s certifications include Diplomat, American Board of Internal Medicine; and Certification in Geriatric Medicine. He is a Clinical Professor in the Department of Medicine at the University of California, San Francisco. He serves on the Board of Directors of the International Psychogeriatrics Association.
Dr. Luxenberg has been volunteering on committees for On Lok since 1988 and has served on the Board of Directors. He came to On Lok in 2011 from the Jewish Home in San Francisco where he served as the Chief Medical Officer. Dr. Luxenberg is the author or co-author of many research publications, book chapters, and three books related to aging. He has been invited to speak at many professional meetings internationally. He served as co-chair of the Alzheimer’s/Dementia Expert Panel for the Mayor’s office, City and County of San Francisco
Gary Campanella, MBH
Chief Financial Officer
Mr. Campanella is responsible for the financial management of each of the On Lok corporations. He began his work with On Lok as interim CFO in January 2016 and became On Lok’s permanent CFO in May 2016. As CFO, Mr. Campanella provides both tactical and strategic oversight in a number of areas: accounting and reporting (internally, Board of Directors, and government agencies); financial planning and analysis; cash management and investments; financial systems; and organizational development.
Mr. Campanella has held senior finance roles in healthcare since 2004 in small to mid-sized, private, for-profit entities – a national behavioral health provider involved in substance abuse treatment; a physician-based pain management company; a regional behavioral health provider focused on serious mental illness; and a national developer and operator of urgent care clinics. Prior to healthcare, he worked in high technology companies in Silicon Valley.
Mr. Campanella holds a master’s degree in business administration from the Fuqua School of Business at Duke University and a bachelor’s degree in psychology from Duke University.
Joy A. Moore, SPHR
Chief Human Resources Officer
Ms. Moore oversees all aspects of human resource management and industrial relations policies, practices and operations. She is also a key driver in implementing On Lok’s performance measurement, professional development and work culture. She is a seasoned HR leader with over 20 years of experience across organizations spanning sizes from 50-1000 employees. Ms. Moore’s expertise includes shaping high-performing cultures at Fortune 500 companies by utilizing OD expertise, implementing progressive technical resources, engaging employees and creating robust development programs that build profitability. She is known for executing strategies that motivate teams and individuals to exceed the corporate objectives through various economic cycles.
Ms. Moore holds a B.A. degree in Management/Marketing from the University of Phoenix, along with numerous HR certifications.
Chief Operating Officer
David Nolan is responsible for planning, organizing and directing day-to-day operations of On Lok’s centers while setting strategies which drive the On Lok Lifeways and PACE programs throughout the Greater Bay Area. He will also provide leadership and support across On Lok’s other programs and services to further its mission of supporting seniors living in the community.
Prior to joining On Lok, David was vice president of the CA Medicare Medi-Cal Program (MMP) at Anthem Blue Cross. He was responsible for managing all aspects of California’s Coordinated Care Initiative in Santa Clara County, which included budgeting, financial reconciliation, vendor and provider contracting, staffing and community engagement. He previously held roles at Alameda Alliance for Health, Chi Partners LLC, and NCB Capital Impact. From 1988-1993, he served as On Lok’s director of fund development, marketing and public relations.
David previously served on the board of directors at Charity Tuesday in Ho Chi Minh City, Vietnam, and was the board president for California Culture Change Coalition. He was also the treasurer of the Alzheimer’s Services of the East Bay, on the steering committee for Alameda’s County Long-Term Care Integration Planning Project, part of the California Community Transitions (CCT) Program Advisory Work Group and on the board of directors for the Pastoral Care Committee in Ho Chi Minh City. He holds a bachelor’s degree in political science from San Francisco State University and attended the Defense Language Institute Southwest.
Chief Development Officer
John Blazek is responsible for executing strategies to develop donors and contributions to support each of On Lok’s business units. He oversees all of the organization’s fundraising activities, which include managing day-to-day operations and monitoring donor efficiency through coordination with staff and the appropriate committees.
Prior to joining On Lok, he was vice president of individual giving and chapter president at the National Multiple Sclerosis Society. The New York-based nonprofit supports leading international research and serves more people with MS than any other organization in the world. He previously held roles at Northwestern University School of Law and at the Archdiocese of Chicago. He currently serves on the executive leadership committee for the San Francisco chapter of Lambda Legal, and was previously on the advisory board at Northwestern University Center on Wrongful Conviction.
He was also a member of the National MS Society Task Force on Diversity. John holds a master’s degree in divinity from the University of Saint Mary of the Lake, a doctor of law degree from Northwestern University School of Law and a bachelor’s degree in art history from Marquette University.
