PACE Model

On Lok Lifeways is a certified Program of All-inclusive Care for the Elderly (PACE).

The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Why PACE was Created

PACE (Program of All-inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home.

PACE Allows Seniors to Remain Living in Their Homes

The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

PACE participants receive in home care services and transportation to a PACE center for primary medical care, social and recreational activities, and other senior care services.

Many Seniors are Eligible for PACE

The PACE model serves seniors who are 55 years of age or older, live in the PACE service area, are certified by their state to need a nursing home level of care, and are able to live safely in the community at the time of enrollment.
