The On Lok Lifeways PACE Program is the Perfect Alternative to a Nursing Home

Our integrated teams of elder care specialists partner with seniors and their families to develop individualized care plans that keep our participants living at home for as long as possible.

We value participants’ independence and self-determination. Many of our professionals are bilingual, and we honor participants’ diverse heritages.

The On Lok Lifeways PACE program may be right if you or a loved one is:

  • 55 years of age or older.
  • Living in San Francisco County, Santa Clara County, or the cities of Fremont, Newark, or Union City in Alameda County.
  • Certified by the State of California to need a nursing home level of care.
  • Able to live safely in the community at the time of enrollment.

Affordable Senior Care

The On Lok Lifeways PACE program accepts Medicare, Medi-Cal, and private payment. All eligible seniors can participate in On Lok Lifeways even if they do not receive Medicare or Medi-Cal benefits. The Medicare and Medi-Cal benefits that you receive will determine your individual monthly cost for On Lok Lifeways.
